I understand and give consent that the Youth Director may contact my child via cell phone call, text message, letter, or email throughout their youth group membership to receive reminders and updates. 

I understand and give consent that my child's photograph may be taken at BaHTY, Jewish Teen Initiative programs, Temple Beit HaYam, and NFTY Regional Events, and that my child may appear on website, brochure, Facebook, or other promotional materials. 

My child may be given over the counter medications (i.e. headache relief medication, cough drops, decongestants, etc.)

I do hereby give my child permission to ride in designated transportation as decided by the Temple Beit HaYam Leadership to BaHTY, NFTY, JTI, or Temple Beit HaYam events. I understand at times there will be events that require transportation operated by a third party (such as a bus company), and do not hold Temple Beit HaYam, NFTY, JTI or Temple Beit HaYam employees or agents responsible for any damage caused by these operators. 
By signing this form, I also understand that youth attending programs must remain at the event or program for the duration unless otherwise arranged with the Youth Director.

I hereby authorize the Temple Beit HaYam Youth Director and Staff to hospitalize or secure proper medical treatment for my child. I agree to indemnify and hold harmless Temple Beit HaYam, its employees, and/or agents for any and all claims and injuries may occur in connection with with my child's Youth Group activities, including direct or indirect acts of negligence on the part of Temple Beit HaYam, its Youth Director, and/or agents.
